Chord changes for Thelonious Monk's "Ba-Lue Bolivar Ba-Lues-Are(Bolivar Blues)". The melody for this always reminds me of Fats Domino's "Blueberry Hill" for some reason.

Printable GIF image of the score for this progresssion. The last chord of the form is notated in the GIF as a Bb13 chord. It is actually a Bb13(#11).

New feature: use the SongTrellis Excerpt Service to customize this piece. Prepare scores for transposing instruments, transpose the score to a new key, change tempo, clip an excerpt for your own practice, study, or composition work.
